DUNF Survey and Instructions
DUNF Forms (Drug Use Notification Form) are required for all marketed animals. This form is your legal declartion that:
- Your animal is free from medication or
- If medications have been administered, the withdrawal period has notyet passed.
Reminder: The DUNF form is a legal document, and you are required to be truthful when completing it.
Market Poultry - see the leg band information below. Use your leg band numbers and enter both chickens on the SAME FORM!
Rabbits - enter correct tattoo numbers for all market rabbits and single fryers on the SAME FORM!
Sheep, Goats, Dairy Beef, Market Beef - enter tag numbers from your pre-fair weigh -ins. Enter these SEPARATELT! This includes Beef and Dairy Beef Feeders.
Swine - exhibitors must wait until weigh-in when they know their ear tag numbers. Each animal must be entered SEPARATELY!
