Make Up Training/kickoff will be Wednesday, February 8 at 5 pm. for those who were unable to attend on January 21.  This is a mandatory required meeting for at least one advisor from every club.  Enrollment packets will be distributed to those in attendance.

4-H FORMS

 Club Charter  

Club Information  - Must be completed by each club and returned BEFORE enrollments packets are distributed

Reenrollment forms - Can be picked up at the office , each returning youth to your club should have a form.  Have youth and parents make sure all information listed is correct and current.  For youth that are project age - project enrollments also go on this page.  PROJECTS MUST BE LISTED.  Forms without projects will not be accepted.  Be sure to have all necessary signatures (youth, parent/guardian and club advisor)  Due April 1.

Membership Registration Form - Must be completed and returned with $4.00 fee for each member and advisor.  If in more than one club, the fee is only due once.

4-H Enrollment Form for new members only - 

4-H News Report - To be completed and returned to Extension Office for publication in the local news paper

4-H Club Meeting or Activity Sheet  - This form is due by August 1 for each club
