High School graduating Seniors should fill out this form and return it to the OSU Extension~Henry County Office by April 1st of the current year.
File 4-H Honor Member Form
4-H members (8 or 9 & in the 3rd Grade through 18 years of age) should fill out this form if they would like to be considered for a 4-H Honor Member award.
File 4-H Silver Spur Award Form
All 4-H members who are enrolled in a 4-H horse project are encouraged to fill out this award form. Deadline for submitting is September 1 of the current year.
File HC 4-H Endowment Mini-Grant Application
Henry County 4-H Clubs are welcomed to write a mini-grant proposal and submitt it by April 1st to the OSU Extension~ Henry County office.

Organizational Advisors can keep track of their club's accomplishments in the various categories of the Honor Club Program.
File HC 4-H All-In-One Club Program Report
This form should be used for reporting the 4-H club's community service, educational programs, Parent/Family Night, Demonstrations and Attendance. Must be submitted by September 1st of current year.

Use this form when members are involved in a field trip or tour or other club activity or an event in which club members will be transported by someone other than the parent.
File General Health Form~ FOR CLUB ACTIVITIES ONLY
This form should be used when a club is conducting a day long or overnight event in which a medical emergency could arise. Member parents should fill out the form in its entirety and return it to the club advisor prior to such activities.
