2012 4-H Camp

4-H Camp Scholarship        PDF

Parents that are seeking financial assistance for their child to attend 4-H camp may request scholarship dollars provided by the Dayton Foundation Harmony Campership Fund and the Clark County 4-H Committee through the Send A Kid To Camp Campaign.  Youth must be age 8 and in 3rd grade to age 14 as of January 1st to apply for 4-H Camp Scholarships.  Youth do not have to be 4-H members to apply for 4-H camp scholarship dollars.  Application deadline May 15, 2012.

All 4-H dogs must have a current vaccination form, dog tags and license.  These documents must be presented at 4-H Dog Skillathon on June 27 in order to be eligible to show in any of the Clark County 4-H Dog Shows or events.

Horse Lease Form    PDF

Any 4-H member that does not own their horse, but is leasing a horse must submit a Horse Lease to the Extension Office by May 1st.  Use the attached guidelines to assist you in drafting a lease that must be signed by the lessee, lessor and the member's parents/legal guardians.  In addition, the horse identification form (sketch) must be submitted with the written lease.

Must be completed by every 4-H club enrolling horse project members.  You must list each member's project horse(s) and the required information.  Members that wish to participate in the Clark County PAS Horse Show must register on this form as well with their classes.  Members may enter in up to 4 classes for PAS with only one horse.  Refer to the 2012 State 4-H Horse Show Schedule for class numbers.  Click Here.

Any horse project member may enter in the Horse Royalty Competition.  This contest is the same contest as the Outstanding Horse Youth, just renamed with different awards.  See contest rules on the form.  

Out of County PAS Request Form       PDF

Any 4-H member that wishes to show in another county's PAS Horse Show to try to qualify for State Fair must complete an Out of County PAS Request Form.  Form must be signed by the 4-H County Extension Educator and faxed to the county where you wish to try out.  A Horse Description Form must also be completed and submitted with the Out of County PAS Request Form, click here.
